MARIE L. ARTISTRY *COVID-19 Requirements*

A COVID-19 Release Form will be sent separately to your email on file. It is required for services to be performed. If you cancel due to signs of COVID, or possibly have been exposed to COVID all fees are still valid associated with scheduled appointments. In the event of a government shutdown, appointments will be rescheduled for a later date.

 Please note that masks are  REQUIRED upon entry at MARIE L. ARTISTRY.

*MARIE L. ARTISTRY Policies & Procedures*

  • A VALID credit card on file is REQUIRED upon booking to secure your selected appointment time. The appointment will be cancelled if a VALID credit card is not on file.
  • A 50% deposit is REQUIRED upon booking for the selected appointment/services if  booked in Spa. This applies to all clients including new, current, and returning clients. All deposits are non-refundable and non-transferable; and goes toward your balance of appointment/service booked at that time.
  • All clients including New, Current and Returning clients are only given a 5-minute grace period however after that you must reschedule and will be charged the remaining 50% of the original appointment/service pricing.
  • In the event of a government shutdown, appointments will be rescheduled. Please give the esthetician Qiana Robinson grace during this period.
  • Reschedules/Cancellations by the client must be made 24 hours BEFORE scheduled appointment. Do not depend on the esthetician Qiana Robinson to cancel and/or reschedule your appointment.
  • Same day cancellation/rescheduling fee is the remaining 50% of original appointment/service pricing. This applies to all clients including new, current, and returning clients!
  • If an appointment is cancelled less than 24 hours the remaining 50% of the appointment/service will be charged via card on file.
  • Clients MUST use the confirmation link that was sent via text/emailed to cancel and/or reschedule appointment.
  • If a client arrives late and the service provider Qiana Robinson has a client immediately following, the same day rescheduling fee is valid.
  • All contracts and forms must be signed before the service provider Qiana Robinson of MARIE L. ARTISTRY performs any services.
  • All appointments done by the service provider Qiana Robinson MUST be confirmed by the client within 48 hrs of receiving notification via text/email.
  • At any time, the service provider Qiana Robinson has all rights to cancel appointments that have not been confirmed by the client within 48hrs of receiving notification via text/email.
  • Clients MUST be over the age of 18 for any advanced services (intimate waxing, advance facials, teeth whitening) without parent or guardian present.
  • Facials and waxing are available for minors ages 12-17, a parent or guardian MUST sign on the minor’s behalf.
  •  No additional guests will be allowed at appointments in the treatment room and or waiting area. That includes children, friends, spouse, boyfriend, girlfriend and or pets that are not a service animal.
  • If you have any signs of any possibly contagious sickness, you will be canceled and charged the remaining 50% of the appointment/service pricing for not canceling or rescheduling.
  • If a client did not notify the service provider Qiana Robinson of MARIE L. ARTISTRY of their late arrival that falls outside of grace period, this is considered a NO CALL NO SHOW (NCNS) and will be charged the remaining 50% of original appointment/service booked.
  • NCNS are charged the remaining 50% of service charge via card on file. If the client confirmed the appointment, did not show up and contacted the service provider Qiana Robinson of MARIE L. ARTISTRY immediately after being charged this is still considered a NO CALL NO SHOW.
  • PLEASE NOTE if the payment card on file declines, MARIE L. ARTISTRY will reattempt charging the form of payment on file for up to 120 days after the date of scheduled appointment. Any overdraft fees are not the responsibility of this business but of the signer of this agreement.
  • In the event of inclement weather, emergency, tragedy for the service provider, or world disaster deposits are still non-refundable and prepaid service plans are nonrefundable.
  •  All appointments will be rescheduled, and deposits go towards appointment only.
  • For any reason, the esthetician Qiana Robinson of MARIE L. ARTISTRY has all rights to refuse service at any time.
  • Once you leave your scheduled appointment you are acknowledging that the service or services performed were completed and all expectations were met. Please keep in mind that results do vary; every face, eye shape, skin concern etc. are different. Although it is impossible to list every potential risk and complication, the client has been informed of possible benefits, risks, and complications of treatment and agrees to service.
  •  Clients must agree to follow post care/aftercare treatment plans and or product recommendations from the service provider Qiana Robinson.
  • In the event of any medical concerns post treatment, the client agrees to contact medical professionals and follow up with MARIE L. ARTISTRY service provider Qiana Robinson.
  •  Client agrees to give an accurate account of medical history, current medications, allergies, and any products used topically or by ingestion. Any open wounds, cuts, sores, etc. on the service area per TDLR, I Qiana Robinson cannot perform any service on said area. Please note that all sales are final; no refunds and/or no exchanges!
